Primary Care at Home, Inc. (PCAHI) was proud to participate in the 3rd Annual Equity & Empowerment Expo, hosted by Caltrans District 4 in Oakland, California. The free public event brought together community organizations, advocates, service providers, and local residents for a day focused on equity, empowerment, and access to resources.

Representing PCAHI at the event were Jesse Brooks, Community Liaison, and Kathryn Hawley, one of PCAHI’s newest Nurse Case Managers, who spent the day connecting with attendees, sharing information about PCAHI’s programs and services, and building new community relationships.

Sharing Resources and Building Connections

Throughout the expo, PCAHI tabled alongside a diverse group of community organizations and collaborating partners, helping attendees learn more about the healthcare and support services available throughout the Bay Area.

Visitors stopped by our table to learn about:

  • HIV and STI testing services

  • PrEP education and prevention resources

  • Medical case management services

  • Community-based healthcare and support programs

The event created an important opportunity to engage directly with community members in an approachable and welcoming environment.

Community Engagement Through Outreach

Our table saw strong engagement throughout the day, with attendees especially excited about PCAHI’s outreach materials and swag giveaways.

We distributed:

  • PCAHI brochures

  • T-shirts

  • Caps

  • Pens

  • Other promotional materials

The giveaways helped spark conversations and created opportunities to introduce new individuals and organizations to PCAHI’s mission and services.

Expanding Awareness Through Community Presence

Events like the Equity & Empowerment Expo are an important part of PCAHI’s outreach efforts. By participating in community-centered spaces, we are able to:

  • Increase awareness of available healthcare services

  • Strengthen partnerships with local organizations

  • Reduce barriers to care

  • Build trust within the communities we serve

We are grateful to Caltrans District 4 for hosting an event that centered equity, empowerment, and community collaboration.

PCAHI in the Community

At PCAHI, we believe healthcare access begins with visibility, trust, and meaningful engagement. Whether through outreach events, testing initiatives, or community partnerships, we remain committed to connecting people with compassionate, culturally responsive care.
