Case Management
At Primary Care at Home, we implement the comprehensive medical case management model. You'll have a team comprising of a nurse and social worker to assist with your medical and psychosocial needs. From doctor's visits and prescription refills to legal and housing issues, you'll be supported every step of the way.
AIDS Medi-Cal Waiver Program
The AIDS Medi-Cal Waiver Program (MCWP) is an immediate assessment and care option for people living with HIV (PLWH) who are unable to continue living on their own without supportive assistance. We provide community/ home-based services as an alternative to nursing facility care or hospitalization.
The AIDS MCWP is funded by the California Dept. of Public Health's Office of AIDS Administration.
Ryan White Home and Community-Based Health Services
As the recipient of the Ryan White Part A Program grant, Primary Care at Home, Inc. has been able to provide home health services, such as skilled nursing, physical therapy, occupational therapy, speech therapy, and health aide services, along with medical case management to PLWH living in Alameda County.
