About Us

Primary Care at Home, INC. (PCAHI) is a non-profit medical organization that provides comprehensive healthcare and case management services, with a particular emphasis on uplifting underserved and marginalized communities. We go beyond traditional healthcare settings, meeting individuals where they are—whether in their homes, community centers, shelters, or on the streets—ensuring no one is left behind. In truth, we pride ourselves on being the Bay Area safety net’s safety net. Central to our mission is our work towards the eradication of the HIV epidemic in the California Bay Area (U=U) and our support of people living with HIV/AIDS (PLWH)

PCAHI's mission is to improve the health and well-being of our community by providing accessible and compassionate healthcare services to people living with HIV/AIDS (PLWHA) and our aging population, with a particular emphasis on uplifting underserved and marginalized communities.
