About the Role

Primary Care at Home, Incorporated (PCAHI) is seeking Therapists to join our inter-interdisciplinary team as consultants. PCAHI is a non-profit community-based organization with offices in Oakland and San Francisco.  PCAHI has a rich history of providing compassionate and innovative team-based care to a diverse community. 

 

The consultants who support our Participants will be part of an integrated care team of Primary Care Providers, Nurses and Social Workers who comprise an interdisciplinary team. The primary focus of our therapists will be psychotherapy and initial assessment of new participants.

 

Candidates must be Board Certified as an PsyF, Psychiatrist, MFT, LCSW, or other related certification. We would prefer at least one-year related experience with multi-cultural, high-risk patients in outpatient settings. The ability to speak Spanish is desirable but not essential. All candidates must be willing to travel to program sites, Participant homes, and/ or conduct telehealth appointments (whatever is most convenient for the Participant).

Reports To: Director of Client Services or Registered Nurse Case Manager

Responsibilities and Duties

Provides mental health assessment, diagnosis, treatment and crisis intervention services for adult and/or family members with a broad range of mental health.  Collaborates with treating physicians, psychiatric and allied health professionals, and the Interdisciplinary team in order to plan and direct each individual Participant’s treatment program.

 

Develops, implements, and documents the psychotherapeutic treatment plan for an assigned caseload of patients, including prompt completion of clinical reminders.  Assumes complete professional responsibility for his/her clinical findings, patient care decisions, and documentation.

Qualifications

  • A minimum of 1 year (preferred) clinical experience with individuals with serious and persistent mental illness.


  • Current license required (LCSW, MFT, PsyD…)


  • Excellent verbal and written skills


  • Understanding of psychiatric diagnosis(es), terminology and medical record charting.

  • Excellent group facilitation skills and experience.


  • Knowledge and understanding of existing relevant statutes, case law, ethical codes, and regulations affecting professional practice of Participant and family therapy. This includes the ability to assist clients in making informed decisions relevant to treatment to include limits of confidentiality


  • Ability to independently assess the psychosocial functioning and needs of patients and their family members, and the knowledge to formulate, implement, and re-evaluate a treatment plan through continuous assessment identifying the patient's problems, strengths, readiness to change, external influences and current events surrounding the origins and maintenance of the presenting issue, and interactional patterns within the client system. This includes the utilization of testing measures where appropriate.


  • Ability to provide counseling and/or psychotherapy services to individuals, groups, couples and families in a culturally competent manner that facilitates change through restructuring and reorganizing of the client system


  • Receives referrals on an assigned medical service; reviews medical and social information; makes assessment and documents findings for Participant’s medical record.


  • Knowledge of psychological testing techniques, administration and interpretation.


  • Excellent interpersonal and communication skills


  • Experience working with dually diagnosed populations and those with acute symptoms of a wide range of psychiatric and chemical dependency diagnosis.


  • Experience with case management and integrated treatment planning.
